Clinic Details:The Rapides Primary Health Care Center began operations as a community based and governed not-for-profit organization in 1996. Rapides Primary Health Care Center has been providing outpatient Medical, Dental, Mental Health healthcare services to our community. RPHCC is a Certified Application Counselor Designated Organization and one of a few health care providers in the community that accepts Medicaid and uninsured patients. RPHCC offers a sliding-fee scale to determine the cost of care for patients without insurance. RPHCC utilizes several programs for under-insured and non-uninsured patients including the Cenla Medication Access Program (CMAP), the LINKS program and the Wellness Program. Rapides Primary Health Care Center is a Federal Tort Claims Act (FTCA) Deemed facility.

Details: EXCELth, Incorporated is a private, not-for-profit organization founded in 1991, governed by a consumer board of directors. EXCELth is funded as a Community Health Center by the Bureau of Primary Health Care, U.S. Department of Health and Human Services and is designated as a Federally Qualified Health Center (FQHC). EXCELth operates the EXCELth Primary Care Network. This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
Details: Access Health Louisiana (AHL) operates eleven school based health centers within Jefferson, Orleans, St. Charles & Iberville parish. These clinics are run in partnership with the parish school systems. Students and staff can get access to high quality care throughout the school year on all campuses. Details: SCHS is a Federally Qualified Health Center which is federally and state funded to provide primary and preventative care in seven rural-based communities in Southeast Louisiana. The twenty-one service sites are conveniently located in East Baton Rouge, Livingston, Tangipahoa, and St. Helena Parishes. As a private, non-profit 501 (c) (3) organization, Southeast Community Health Systems offers medical, dental, and behavioral health services, which include mental health counseling, substance abuse treatment, obstetrics and gynecology, and podiatry services. Through health education, Southeast Community Health Systems work to empower patients to guide their own health care by working collaboratively with our providers. Details: Calcasieu Community Clinic is a not-for-profit, free health care clinic located at McNeese State University’s Juliet Hardtner Hall College of Nursing. The clinic provides free ambulatory medical care and pharmaceuticals to the low-income, working uninsured in the five parish area of Imperial Calcasieu including Allen, Beauregard, Calcasieu, Cameron and Jefferson Davis.
Details: The Mission of the MLK Health Center & Pharmacy is to extend comprehensive primary healthcare and pharmacy services at no cost to uninsured patients with chronic illness who otherwise often forgo routine medical care because of a lack of resources.
Details: CareSouth is a private non-profit Federally Qualified Health Center that provides a comprehensive range of community-based medical, dental and behavioral health services. Our health center is guided by a consumer-majority volunteer board of directors whose mission is to provide comprehensive, high quality health care with compassion and respect for all. Our experienced team provides a broad range of medical and dental services at our main location in Baton Rouge and in satellite clinics located in Donaldsonville and Plaquemine, Louisiana. CareSouth also provides onsite behavioral services in two Ascension Parish public schools. CareSouth is certified by the National Committee for Quality Assurance (NCQA) as a Level 3 Patient-Centered Medical Home. CareSouth originally launched in 1997 as Capitol City Family Health Center (CCFHC), which it operated under until changing its name in April, 2016 to be more inclusive of the communities we serve.
