Clinic Details:Signature Health was founded in 1992 by our CEO, Jonathan Lee, Vice-Chair of the Board of Directors, Paul Brickman, and COO, Ann Mason. We began as a community-focused organization, providing counseling to kids in local schools. Today, Signature Health is a non-profit Federally Qualified Health Center. Rooted in our local communities as we have always been, we now thrive as a full-service health care agency. Our services range from counseling, to alcohol and drug recovery programs, to primary care.
Details: AxessPointe Community Health Centers, originally named Akron Community Health Resources, Inc., opened its doors in 1995 after receiving funds from the Bureau of Primary Health Care to establish the first federally qualified health center in Summit County. AxessPointe has five current sites in Northeast Ohio, including three in Akron, one in Kent and one in Barberton. As an FQHC, we deliver primary medical and dental care in medically underserved areas. AxessPointe also provides behavioral health, women’s health and pharmacy services. While our focus is on uninsured, underinsured and Medicaid/Medicare patients who may not have access to affordable health care, our services are welcome to all. Our health care teams provide diagnosis, treatment and preventive medical and dental services. We accept Medicaid, Medicare, most private insurances and provide a sliding-fee scale based on family size and income. AxessPointe Community Health Centers is a Federally Qualified Health Center (FQHC), represented by the OACHC. FQHCs receive operating grants under Section 330 of the United States Public Health Service Act. In order to comply with federal law, FQHCs must be established as a not-for-profit corporation and the majority of the board of directors must be consumers of the services provided by the facility. The OACHC, Ohio Association of Community Health Centers, is a non-profit organization that represents Ohio’s FQHCs. The mission of the OACHC is to ensure high-quality affordable health care for all Ohioans through the growth and development of Ohio’s Community Health Centers. As a National Committee of Quality Assurance (NCQA) certified patient-centered medical home (PCMH), we are committed to high-quality care and service. We are also a member of the National Health Service Corps – which ensures we have caring, qualified and passionate health care providers through loan repayment packages.

Details: ONE Health Ohio’s Newton Falls location, Falls Family Care, is a Patient-Centered Medical Home Recognized Health Center that offers integrated healthcare services. Services include primary medical care, dental care and behavioral health care, as well as on-site lab testing. The health center services all ages, including adults, teens, children and infants.

The Danville location of the Knox County Community Health Center is a patient-centered medical home offering preventative and primary care to patients of all ages. Details: HealthSource Lebanon Family Practice is located on Columbus Avenue, in the old Blockbuster store, near Kroger. Our Family Practice providers treat patients of all ages. They provide comprehensive and continuing care for both males and females. Your Family Practitioner may treat you and your family members for acute conditions such as a minor injury, the flu, or a sore throat. He or she may also treat patients for chronic conditions such diabetes or high cholesterol.
Details: HealthSource West Clermont is a School Based Health Center that also serves the entire community. The services provided at HealthSource West Clermont include primary medical care, vision services, and dentistry to any West Clermont School District student, school district staff, and the entire community. The school-based health center is open Monday through Friday and will remain open during school breaks including the summer. You don't need to live in the West Clermont District or have a child enrolled at West Clermont Schools to use the West Clermont SBHC - it is open to the public.
