About this clinic

Community Health of South Florida Inc. is a nonprofit health care organization providing affordable quality primary and behavioral health care services to the residents of rapidly growing South Florida. CHI operates 11 state-of-the-art primary care centers and 31 school-based programs. All centers offer quality comprehensive primary and behavioral health care services. Our physicians are board certified or board eligible. Additionally, CHI is accredited by the Joint Commission and is accredited as a Level 3 Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA). CHI offers a “one-stop-shop” model where the organization and delivery of quality services are seamless, affordable, accessible and culturally sensitive.

My appointment was 5 30pm and I arrived 5 15pm. I ran out of work early to be on time. I wake up at 6 am and lunch at 12:30. It was 7:30pm and the doctor was still starting to see a patient ahead of me. I was exhausted, hungry and I consider this a lack of respect and consideration for the patient. Doctors are not GODS, they are employees and depend on patients to get jobs and pay their salaries. Very poor customer service.

Is this clinic actually free?
Often not entirely. "Free clinic" usually means one of three things: a volunteer clinic with no charge, a community health center with a sliding fee scale based on household size and income, or a clinic that charges a small flat visit fee. A sliding scale can still leave a bill. Ask what the lowest tier costs before you book.
What if I have no income at all?
Most sliding-scale programs have a lowest tier for people with no income, and many accept a signed self-declaration when you have no pay stubs to show. Ask for the clinic's discount program by name and ask what proof they take. Being told "bring proof of income" does not mean you are turned away for having none.
What should I bring to a first visit?
Usually a photo ID, something showing your address, proof of income or a self-declaration, your insurance or Medicaid card if you have one, and a written list of your current medications and doses. Call ahead — requirements vary, and arriving without the right paperwork is the most common reason a first visit gets rescheduled.
Do I need insurance, or immigration documents?
Federally funded health centers serve patients regardless of insurance status and do not require proof of citizenship to provide care. Volunteer and charitable clinics set their own rules. If it matters to you, ask before your visit what they require and what they record.
Is dental care included?
Usually not, and this is the single most common disappointment. Dental is frequently a separate program with its own eligibility, its own waiting list, and a narrower service list — extractions and urgent pain may be covered while fillings, crowns, root canals, and dentures are not. Confirm that dental is offered at this specific location before travelling.
Can I get a prescription refilled here?
Most clinics require an appointment before they will refill anything, and a first appointment can be weeks out. If you are close to running out of a medication you cannot safely stop, say so when you call — say the medication name and how many days you have left. Ask your pharmacy at the same time; pharmacists can sometimes authorise a short emergency supply.
What if I need care today?
