Clinic Details:Sunset Community Health Center, Inc. is a migrant and federally qualified community health center, receiving Sec on 329 and 330 funding through the Public Health Service to provide health services to the residents of Yuma County. Sunset is a non-profit, 501(c)(3) organization governed by a 12 member Community Board of Directors whose primary Mission is to deliver comprehensive and preventive primary care in a culturally sensitive, compassionate and professional manner, ultimately improving the health of our patients and responding to the health and education needs of our communities with the Vision to build healthier communities and to improve the quality of life those we serve by collaborating and partnering with other providers and community organizations. Our all-volunteer community health centers Board of Directors are made up of community leaders serving our current patient population. The management of Sunset Community Health Center is comprised of a Chief Executive Officer, Chief Medical Officer, Chief Financial Officer, Associate Medical Director, Dental Director and South County Director. Quality Assurance is maintained through a multi-disciplinary committee meeting once per month chaired by the Chief Medical Officer.

Details: Sunset Women’s Medical Group offers a full range of gynecologic and obstetric health care for women at all ages and stages of life. Regular visits with a women's health care provider builds a foundation of health and wellness for women of all ages. The purpose of an annual well woman visit is to promote wellness, screen for potential health problems, and manage any ongoing conditions, in addition to helping prevent future illnesses from developing.

Details: The School Health Care Centers provides primary care and dental services to children and their siblings: Who do not qualify for AHCCCS or KidsCare; Who have applied for AHCCCS or KidsCare and are waiting to be notified; Who meet the age requirements (must be under 14 years old and or currently attending 8th grade); Siblings of students attending school might be eligible for the program; All qualified children are eligible even if the program is not stationed in the school where your child is currently attending.
