TAN Healthcare is a Federally Qualified Health Center(FQHC) created in 1987 as a Triangle AIDS Network. TAN was created by a group of concerned citizens in response to the growing health crisis of acquired immune deficiency syndrome or AIDS. The changing landscape of healthcare has led us to expand our healthcare endeavors to include all care for all people. As national trends are mainstreaming HIV care, TAN has transformed into a full-service clinic providing comprehensive and primary care services for all individuals in Southeast Texas, regardless of their ability to pay. We are dedicated to serving everyone in Southeast Texas including the underinsured and uninsured. This health center is a Health Center Program grantee under 42 I.S.C. 254b that receives HHS funds and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n) with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

Details: Avenue 360 Health and Wellness, a 501 (c)3 organization, is a Federally Qualified Health Center with seven clinics in the Houston region providing primary, behavioral health, and dental care to medically underserved communities. Previously, the organization was known as Houston Area Community Services (HACS), founded in 1998 to treat those living with AIDS. Several years ago, HACS merged with Bering Omega (Omega House), which provided hospice care to people living with HIV/AIDS, to form Avenue 360. The mission of Avenue 360 is to provide high quality and caring services to promote healthy people and communities.
Details: Spring Branch Community Health Center (SBCHC) is a private, non-profit community health center providing quality, affordable healthcare services to the underserved and uninsured communities of Spring Branch and West Houston. SBCHC was developed to reduce area health disparities by creating a neighborhood-based health center and by providing comprehensive primary health care services that are culturally competent, linguistically appropriate and client-driven in a location easily accessible to public transportation. SBCHC was formed in 2003 and began providing comprehensive health care services to indigent and low-income patients in the Spring Branch community in May 2004. Designated as a Federally Qualified Health Center (FQHC) in September 2005, SBCHC offers medical care, mental health, dental care to both children and adults, no matter what your income.
Details: St. Hope Foundation Inc. is a nonprofit 501(c)(3) community healthcare organization that was established in November 1999. St. Hope is recognized as a Patient-Centered Medical Home by the National Committee of Quality Assurance (NCQA). Our organization was created in response to the growing need for innovative, effective healthcare designed to provide quality services to reduce health disparities within Houston, Texas and rural counties. We serve patients from various disease states that include, but is not limited to, heart disease, diabetes, HIV/AIDS, Hepatitis C, obesity, high blood pressure, obesity, gout, skin disorder, cancer, and mental health. As a comprehensive, federally-qualified Health Center (FQHC), St. Hope targets unmet needs and gaps in health-related services, fostering client-centered programs to meet these necessities. Within the last 20 years of service, St. Hope has performed routine health screenings for more than 30,000 people and has been a key prevention contractor for the City of Houston, DHHS, Bureau of HIV/STD and Viral Hepatitis Prevention, and Centers for Disease Control (CDC). Prevention and wellness are essential guideposts at St. Hope. Known for conducting large community health promotions in resource-limited areas, St. Hope has garnered support both locally and nationally for its innovative sexual health screenings with BET’s College Road Tour Rap-It-Up campaign, targeting young African Americans and Latinos at historically black colleges and universities (HBCUs). The St. Hope Foundation receives funding through several financing streams, including HRSA, Ryan White, Centers for Disease Control, DSHS, City of Houston, The Simmons Foundation, and several private foundations.
