For many individuals in the communities we serve, access to affordable and appropriate health care is limited. Our mission is to provide low-income, uninsured, and under-insured individuals with high quality health care. We believe that everyone in our community is benefited as access to appropriate and affordable health care is provided to those who face the most significant barriers to care. We focus on providing this care to the medically underserved while treating each individual we serve with dignity, respect, and understanding. We also encourage our patients to participate in their own health care through education and health promotion.
Details: We are a not-for-profit Community Health Center providing medical, dental and behavioral health care in neighborhoods throughout the northern portion of the Navajo Nation and southeastern Utah. We provide primary health care to men, women, and children, regardless of citizenship status, nationality or ability to pay.

Midtown’s Logan Community Health Center was opened in January 2016 to serve the underserved residents of Cache Valley and the surrounding areas. This site was opened with the support of Intermountain Healthcare to ensure that low-income and uninsured community residents have access to comprehensive primary healthcare services including well care, prenatal care, pediatrics, immunizations, women’s health, family planning, geriatrics, and treatment for acute and chronic illnesses.
