Clinic Details:The West Oakland Health Council is a non-profit Community Health Center providing primary care, mental health, and substance abuse recovery services to residents of West, North and East Oakland, Emeryville and Southwest Berkeley. Details: Our School-Based Health Centers provide age-appropriate comprehensive medical, health education, and youth development services to youth during and after school hours. Our services are aimed at helping students come to school ready to learn and exceed at school. Clinics are open to all students, regardless of income or healthcare coverage.

Details: Located on the east side of Lake Merritt, Frank Kiang Medical Center offers comprehensive health care services for patients of all ages. The center has 10 exam rooms and can serve more than 5,000 patients every year. The modern facility has unique features like larger exam rooms which accommodates our patients who often come to appointments with family members or interpreters. The center is named after Frank Kiang, a community leader and former chairman and CEO of Metropolitan Bank who passed away in 2007.
Details: Mobile Health provides urgent care, brief social work assessments, and referrals to community resources while visiting shelters, meal programs, and other accessible locations in the community. Staff from Alameda Health System and Health Care for the Homeless include a medical provider, medical assistant, social worker, registration clerk, and van driver who work together to provide friendly and accessible care to those who need it most, as well as linkages to ongoing primary care at Alameda Health System or other community clinics.
Details: AMC is the oldest and largest of the Asian Health Services sites and serves over 16,000 patients with nearly 60,000 visits annually. Our team of doctors, nurse practitioners, physician assistants, nurses, health coaches, behavioral health specialists, and nutritionists provide high quality, comprehensive care in 9 Asian languages and English for patients of all ages.
Details: Opened in 2003, the AHS dental clinic is a modern facility equipped with state-of-the-art digital radiology technology. The clinic provides a comprehensive range of primary oral health care services with a strong emphasis on prevention and education.
Details: LifeLong TRUST Health Center recently opened, serving adults with disabling conditions and histories of homelessness who also participate in Alameda County’s General Assistance Program. The mission of the TRUST Health Center is to improve the lives of clients by providing high-value care that addresses clients’ physical, mental, and social well-being in one location. This collaboration between multiple government agencies and non-profit organizations will promote dignity, respect, and compassion in all interactions among staff and clients. Participating organizations will collaborate closely to support clients to find and maintain stable homes, to promote their overall health and well-being, and to develop or re-establish strong social support networks outside of the public services system.
