Like the other two Holy Cross Hospital primary care health center locations, services at this center are provided by a team of medical experts including physicians, nurse practitioners, physician assistants and registered nurses. Services include annual screenings, chronic disease management, behavioral health, preventive care, health education, and follow-up care for emergency room and hospital visits. This health center participates in Montgomery Cares, a public/private partnership that provides health care to low-income, uninsured Montgomery County residents through a network of clinics. Requirements for eligibility to Montgomery Cares are that a person be 18 years or older, have income under 250 percent of the federal poverty level, and live in Montgomery County

Both appointments and walk-in care are available. Community partnerships make it possible to reach underserved populations, and to provide diagnostics, integrated behavioral healthcare and specialty care.

    • Details: The core mission of the Muslim Community Center (MCC) is to serve the community. MCC Medical Clinic is one facet of this commitment. MCC Medical Clinic’s mission is to provide compassionate and high quality medical care to uninsured, indigent adult residents of our community regardless of race, religion, ethnicity, or national origin. To be seen at the MCC Clinic the patients must meet the following criteria: Must be resident of Montgomery County; Have no medical coverage; Income level below 250% of Federal Poverty Line. You will be required to produce a copy of your government issued ID and proof of Income at your initial visit.
