Opened in 2014, the 53,192 square-foot Health Center replaced the former Stout Street Clinic, an aging structure that lacked adequate space to effectively meet the complex healthcare needs of an expanding homeless population. The Health Center introduces a unique model of integrated health care targeted to the needs of homeless patients. It fully incorporates patient-centered, trauma-informed medical and behavioral health care, substance treatment services, dental and vision care, social services and supportive housing to more fully address the spectrum of problems homeless adults and children bring to their medical providers. The Health Center offers a range of health services in one suite, from one team who work closely with each other to care for our patients. Services are provided to individuals experiencing homelessness or at-risk of becoming homeless regardless of a person’s ability to pay or immigration status. As a Federally Qualified Health Center (FQHC), the Colorado Coalition for the Homeless’ (CCH) health care system is eligible for Federal Tort Claims Act (FTCA) medical malpractice liability protection. CCH is a Health Center Program grantee under 42 U.S.C. 254b and deemed a Public Health Service employee under 42 U.S.C. 233(g)-(n).

    • Details: Rocky Mountain Youth Clinics (RMYC) was established in 1996 by Dr. Larry Wolk, a Denver pediatrician who recognized a gap in the Colorado healthcare system that left many children without access to healthcare. Our mission is to provide accessible, affordable, high-quality health care and support programs to infants, children, and youth across Colorado regardless of their insurance status or family’s ability to pay. We have grown from a single clinic that provided approximately 2,000 patient visits a year to a vast statewide network with 12 clinics that provide more than 64,000 visits to over 24,000 patients a year. Our dedicated and highly trained staff strive to provide a true medical home experience tor patients and a welcoming and culturally responsive environment for children and families of all backgrounds. More than 85% of the children we serve live in low-income households and every year, we provide over $1 million of free care to families who are uninsured or unable to afford health care services. We treat our families with respect, compassion and understanding. In 2016, RMYC celebrated 20 years of connecting kids to healthy lives with medical, dental, and behavioral health care, nutrition and fitness education, emergency and supplemental food, early literacy and school readiness support, and linkages to community resources for other basic needs such as housing, transportation, food, and clothing.     • Details: Urban Peak has a 40-bed overnight shelter for youth ages 15 through 20. We are open 365 days a year, 24 hours a day, and offer a safe and welcoming environment where we respect the rights and identities of the youth we serve. We are a safe place for LGBTIQ youth and accept their orientation and identity. While youth are at the Shelter, they will work with a case manager to identify strengths and to create an individualized service plan based on the principles of trauma-informed care to move toward safe and stable housing. This may include providing youth with family mediation services or help accessing their own apartment through Urban Peak. At the Shelter, we offer caring, supportive staff; a medical clinic three half-days a week that gives access for vaccinations, HIV and pregnancy testing, specific health screenings, and more; three meals a day and snacks in between; group activities such as mountain bike excursions, hiking, sports, art and referrals to additional requested services.

    • Details: The Webb Center for Primary Care, located on Denver Health’s main campus, is a large outpatient facility that houses primary care clinics for adults and children, as well as a dental clinic and pharmacy. The center is certified as a Patient-Centered Medical Home, which means patients take an active role in their health care and have access to care that is coordinated between different services. The Webb Adult clinic offers medical services to adults 19 and older - to meet all your primary care needs, as well as help with family planning and care for adults over 65. The Webb Pediatrics clinic provides primary care services for newborns up to 18, and is staffed by a care team that understands the needs of children. Also at Webb Pediatrics, expecting mothers can receive breastfeeding guidance from a pediatrician and nurse who specialize in nursing. The Level One Physicians Clinic, also a part of the center and located on the first floor, offers adult internal medicine and family medicine services for patients of all ages.

    • Details: The Women’s Mobile Clinic is one of Denver Health’s federally-qualified community health centers. The Women’s Mobile Clinic provides patient education as well as cervical and breast cancer screening, including digital mammography. The Mobile Clinic is designed to help reduce barriers to cancer screening that many women experience. By moving the Mobile Clinic to each of the other eight Denver Health community health clinics and to various other sites in the metropolitan area, Denver Health patients are able to more easily access breast and cervical cancer screening. Women may access both cervical and breast cancer screening in one visit or they may opt to have only one service. Our clinic staff is fluent in English and Spanish and for most other languages, interpreter services are available. Patient navigation and case management services are available to those women who need additional follow-up to their screenings.
