- Clinic Details: In 2008 Valley-Wide Health Systems, Inc. developed the convenient care model and established Convenient Care Community Clinic to give patients and the community an alternative to the local emergency room for acute and non-urgent care needs outside of traditional clinic hours. Convenient Care Community Clinic (CCCC) originated as part of the Sierra Blanca Medical Center, but was later relocated to a separate facility at 1131 Main Street in Alamosa. The CCCC model not only offers patients a convenient option for illnesses outside of regular office hours, for outpatient treatment, minor injuries, and/or acute illness, but also provides better access to walk-in, same day services which are available 7 days a week including holidays. CCCC serves as an alternative to the cost of an emergency room visit for problems on the lower end of the severity spectrum. Visits should be for acute care only and should not replace visits to your Primary Care Provider or regular physician. Refills for chronic medication prescriptions cannot be requested through this clinic. CCCC also offers FREE HIV screening services and a full spectrum of STD testing and treatment in a safe and confidential setting. CCCC does not require that you be a Valley-Wide Health Systems patient, and is open to both the community and to visitors needing services while in the San Luis Valley. After your visit, treatment or screening, our Case Managers can help coordinate follow-up care with your Primary Care Doctor or help you select a Primary Care Provider. CCCC is located within walking distance to both the Adams State University and Trinidad State Junior College Campuses and Walgreens pharmacy. CCCC accepts most insurance plans, but requires that all patients bring their insurance information and/or their insurance card.
