Clinic Details:In 2008 Valley-Wide Health Systems, Inc. developed the convenient care model and established Convenient Care Community Clinic to give patients and the community an alternative to the local emergency room for acute and non-urgent care needs outside of traditional clinic hours. Convenient Care Community Clinic (CCCC) originated as part of the Sierra Blanca Medical Center, but was later relocated to a separate facility at 1131 Main Street in Alamosa. The CCCC model not only offers patients a convenient option for illnesses outside of regular office hours, for outpatient treatment, minor injuries, and/or acute illness, but also provides better access to walk-in, same day services which are available 7 days a week including holidays. CCCC serves as an alternative to the cost of an emergency room visit for problems on the lower end of the severity spectrum. Visits should be for acute care only and should not replace visits to your Primary Care Provider or regular physician. Refills for chronic medication prescriptions cannot be requested through this clinic. CCCC also offers FREE HIV screening services and a full spectrum of STD testing and treatment in a safe and confidential setting. CCCC does not require that you be a Valley-Wide Health Systems patient, and is open to both the community and to visitors needing services while in the San Luis Valley. After your visit, treatment or screening, our Case Managers can help coordinate follow-up care with your Primary Care Doctor or help you select a Primary Care Provider. CCCC is located within walking distance to both the Adams State University and Trinidad State Junior College Campuses and Walgreens pharmacy. CCCC accepts most insurance plans, but requires that all patients bring their insurance information and/or their insurance card.

The Alamosa Family Medical Center (AFMC) is part of Valley Wide Health Systems, Inc. which was established in 1976. This clinic and the Valley Wide Pharmacy are centrally located in Alamosa, directly across the street from the Adams State University Nielsen Library, and serve all surrounding communities of Rio Grande County, Conejos County, Costilla County and Saguache County. AFMC offers family practice care for all members of your family no matter what age. The clinic specializes in Obstetrics/Gynecology (OB/Gyn) to care for women before and after childbirth, continuing with pediatric care for your newborn through all developmental stages. Through the use of special programs like Bright by Three and Reach Out and Read promotion for early childhood literacy and a love of books and reading becomes a standard part of early childhood primary care and has been offered in Valley-Wide clinics since 1998. AFMC offers ancillary services for breast feeding education, and provides a private lactation support room that is available to the general public. Family Planning services are also available to help you decide what is “best” for your family planning goals. We offer easy access to FREE pregnancy testing and confidential adolescent visits. FREE HIV screening services and a full spectrum of STD testing and treatment are available in a safe and confidential setting. We take a team based approach to your care. Your primary care provider, nurse team and support staff will ensure a pleasant visit and will provide care for your acute, chronic, preventive, reproductive and behavioral health needs. We can also offer immigration medical exams. After your visit, our case managers can help you select a Primary Care Provider and/or help you look into some insurance or health coverage options if you do not currently have insurance. They can also evaluate you for Medicaid eligibility. All Valley-Wide Health Systems sites are now on an Electronic Health Records system. This allows for immediate access to your health records in any other Valley-Wide medical, dental, physical therapy or anti-coagulation clinic. Valley-Wide Health Systems will also send your prescription directly to your chosen pharmacy electronically when allowable.

Details: Sierra Blanca Medical Center is located on the 3rd floor of the south wing of the San Luis Valley Health Hospital, within walking distance to Adams State University. The clinic was first opened in 1996 and has undergone several changes to enhance patient care since that time. Sierra Blanca Medical Center (SBMC) provides care to the city of Alamosa, Colorado, and surrounding areas including the rural population Alamosa County, the towns of La Jara, Monte Vista, Hooper, Blanca and Fort Garland. Our clinic serves patients of various backgrounds and socio-economic statuses, including: low-income families and specialized assistance for our migrant patients. At SBMC we not only provide primary care, but we also have integrated rehabilitation/physical therapy services available. A full range of Chronic Care services are offered, such as asthma care, to provide literature and educational material so you can fully understand how to successfully manage your condition. We also offer both childhood and adult vaccinations. Being located within the hospital campus, Diagnostic, Laboratory, and Emergency Services as well as the Valley-Wide Pharmacy are easily accessible for our patients. Family Health & Planning services are also available to provide you with options based upon individual beliefs and needs. Valley-Wide Health Systems has partnered with San Luis Valley Behavioral Health Group to provide on-site Behavioral Health services, which are offered Monday through Friday. Our clinic also offers FREE HIV screening services and a full spectrum of STD testing and treatment in a safe and confidential setting. We have various doctors, physician assistants, nurse practitioners and physical therapists on hand to meet the needs of our patients including, after hours assistance, through a Telephone Triage Service. Details: Valley-Wide Health System’s, Heart and Eagle mobile clinic was acquired in 2007. The mobile clinic consists of a full medical exam room and a full dental operatory, although each room can be utilized for either function. Heart and Eagle creates a mobile single point of entry to address medical, dental and social service needs, outside of the traditional clinic setting. Heart and Eagle provides primary and preventative medical and oral health services; focusing on acute, chronic and preventative healthcare services. Outreach activities focus on screening for conditions such as hypertension, diabetes, and cancer. Services include basic vital signs checks, point of care testing, health education and lifestyle modification with specific attention to self-management strategies, limiting environmental risk exposure and harmful health habits. The mobile Heart and Eagle clinic has also been identified as a resource in Valley-Wide’s coverage area in the event that additional medical care facilities are needed in the community or to ensure continuity of care for our patients in the event that any of our clinics could not be opened.
Details: The Alamosa Dental Clinic provides General Dentistry services. Our goal is to provide you with information to guide you through your dental health decisions and comprehensive care. We offer diagnostic services like routine exams, x-rays, and dental screening. Restorative care includes both composite and amalgam fillings depending on patient preference and suitability. Preventive care includes routine cleanings, oral hygiene education, fluoride application, and sealants to prevent decay in deep grooves of the molars. We treat the whole family; including babies, children, adolescents and adults. Pediatric services we provide include restorations, space maintenance, preventive care, and Headstart screenings. Sometimes patients will need more than routine care. Periodontal treatment includes deep cleanings called Scaling and Root Planning to stop bone loss from hard deposits and bacteria below the gum. We also offer Oral Surgery services like extractions, surgical extractions, and alveoloplasty (smoothening the bone to prepare for a denture). Endodontic treatment is provided to patients who have a tooth that can be saved, however the tooth has a cavity into the nerve. This is also called a root canal. Prosthetic treatment includes crowns, fixed bridges, complete dentures, and removable partial dentures. Implants may also be an option in specific cases. When specialty dental care is recommended we are happy to help you explore those options. In the event you have a dental emergency, walk-in times are available for evaluation. You could receive same day treatment when appropriate. The Dental Department also has patient support services. Our Patient Navigator staff will help you with planning your treatment priorities, exploring financial options, and follow-up with treatment and referrals. Our Dental Insurance Support staff coordinates dental insurance benefits and pre-authorizations.
Details: The Alamosa South Health Center (ASHC) is part of Valley-Wide Health Systems, Inc. (Valley-Wide) which was established in 1976. This clinic, opened in October 2017, is located on the campus of the SLV Behavioral Health Group on County Road 9 South in Alamosa. It serves all the surrounding communities in Rio Grande County, Conejos County, Costilla County and Saguache County. Valley-Wide and the San Luis Valley Behavioral Health Group have worked closely together to bridge the physical health and the behavioral health services we provide. This integration project to improve access and effectiveness of healthcare services in the San Luis Valley began in 2009 with co-located services in Valley-Wide clinics. It is exciting to expand integrated services through this new model at the Alamosa South Health Center. The core of this approach is a behavioral health consultant working with a primary care provider in a system that supports team-based care. It will provide care coordination and support to individuals in meeting their self-management goals. The behavioral health consultant works with a primary care provider to evaluate the mind-body-behavior connection and provide brief, solution-focused interventions. ASHC offers family medicine care for all, no matter what age. The primary care provider, nurse team and support staff will ensure a pleasant visit and will provide care for your acute, chronic, preventive and reproductive health needs. Care Coordination, Case Management and Financial Eligibility staff will be available to complete comprehensive needs assessments to identify and overcome barriers to care. Financial assessments for Medicaid, CHP+, Colorado Indigent Care Program (CICP) sliding fee discount program (SFDP) and other health insurance, as well as, health wellness programs will be offered at ASHC. Valley-Wide and the San Luis Valley Behavioral Health Group are committed to working together to strengthen community partnerships to ensure quality and timely integrated services.
