Details: Since 1978, as others closed their doors to those without private health insurance, we have continued to open ours. From a single clinic we have now expanded into a network of health care facilities along the I-70 corridor from Parachute to Avon, and along the Roaring Fork Valley to Basalt. As a nonprofit, community-led, Federally Qualified Health Center system, we are dedicated to providing comprehensive, integrated health care for all. We firmly believe health care is a basic human right, not a privilege. Mountain Family currently serves more than 18,000 patients. Our system integrates medical, dental and behavioral care - a necessary measure for the vulnerable, who often show up in our clinics with medical problems related to poor teeth, substance abuse, trauma, depression, anxiety and other complicated combinations. We exceed national clinical outcome goals for childhood immunizations; hypertension control; tobacco screening and cessation; healthy weight screening, nutrition and activity counseling; depression screening and follow-up; early entry into prenatal care; and healthy birth weight babies. We were also recently rated Colorado’s top performing community health system for diabetes control.

Details: The Alamosa Dental Clinic provides General Dentistry services. Our goal is to provide you with information to guide you through your dental health decisions and comprehensive care. We offer diagnostic services like routine exams, x-rays, and dental screening. Restorative care includes both composite and amalgam fillings depending on patient preference and suitability. Preventive care includes routine cleanings, oral hygiene education, fluoride application, and sealants to prevent decay in deep grooves of the molars. We treat the whole family; including babies, children, adolescents and adults. Pediatric services we provide include restorations, space maintenance, preventive care, and Headstart screenings. Sometimes patients will need more than routine care. Periodontal treatment includes deep cleanings called Scaling and Root Planning to stop bone loss from hard deposits and bacteria below the gum. We also offer Oral Surgery services like extractions, surgical extractions, and alveoloplasty (smoothening the bone to prepare for a denture). Endodontic treatment is provided to patients who have a tooth that can be saved, however the tooth has a cavity into the nerve. This is also called a root canal. Prosthetic treatment includes crowns, fixed bridges, complete dentures, and removable partial dentures. Implants may also be an option in specific cases. When specialty dental care is recommended we are happy to help you explore those options. In the event you have a dental emergency, walk-in times are available for evaluation. You could receive same day treatment when appropriate. The Dental Department also has patient support services. Our Patient Navigator staff will help you with planning your treatment priorities, exploring financial options, and follow-up with treatment and referrals. Our Dental Insurance Support staff coordinates dental insurance benefits and pre-authorizations.

Details: Rocky Mountain Youth Clinics (RMYC) nine School-Based Health Clinics (SBHC) provide comprehensive, on-site health care services to students who do not have a medical home or are in need of health services in a convenient setting. Each SBHC is hosted by a school, and services are available to all students enrolled within the same school district as the host school. In 2015, SBHCs served more than 7,000 patients.

Details: Mission Medical Center (MMC) is here to provide health, hope, and spiritual support. Qualified patients receive integrated health services from licensed professionals in a Christ-centered environment. Mission Medical Center works with the health care community in the Pikes Peak Region to identify specialists to provide referral services on a case-by-case basis. Mission Medical Center patients receive outpatient medical and behavioral health care for chronic medical illnesses at no cost. Patients typically remain under care at MMC for 10 to 20 months. MMC coordinates continuing care with other safety-net providers, particularly Peak Vista Community Health Center. Patients typically are 19-64 years old, have no form of health care coverage, and with income below 250% of the Federal Poverty Guidelines; $29,425 for a household of one person, a family of 4 is $60,525. MMC also cares for the under-insured, people that have insurance, but high deductibles.
