Details: Since 1978, as others closed their doors to those without private health insurance, we have continued to open ours. From a single clinic we have now expanded into a network of health care facilities along the I-70 corridor from Parachute to Avon, and along the Roaring Fork Valley to Basalt. As a nonprofit, community-led, Federally Qualified Health Center system, we are dedicated to providing comprehensive, integrated health care for all. We firmly believe health care is a basic human right, not a privilege. Mountain Family currently serves more than 18,000 patients. Our system integrates medical, dental and behavioral care - a necessary measure for the vulnerable, who often show up in our clinics with medical problems related to poor teeth, substance abuse, trauma, depression, anxiety and other complicated combinations. We exceed national clinical outcome goals for childhood immunizations; hypertension control; tobacco screening and cessation; healthy weight screening, nutrition and activity counseling; depression screening and follow-up; early entry into prenatal care; and healthy birth weight babies. We were also recently rated Colorado’s top performing community health system for diabetes control.

Details: The Alamosa Dental Clinic provides General Dentistry services. Our goal is to provide you with information to guide you through your dental health decisions and comprehensive care. We offer diagnostic services like routine exams, x-rays, and dental screening. Restorative care includes both composite and amalgam fillings depending on patient preference and suitability. Preventive care includes routine cleanings, oral hygiene education, fluoride application, and sealants to prevent decay in deep grooves of the molars. We treat the whole family; including babies, children, adolescents and adults. Pediatric services we provide include restorations, space maintenance, preventive care, and Headstart screenings. Sometimes patients will need more than routine care. Periodontal treatment includes deep cleanings called Scaling and Root Planning to stop bone loss from hard deposits and bacteria below the gum. We also offer Oral Surgery services like extractions, surgical extractions, and alveoloplasty (smoothening the bone to prepare for a denture). Endodontic treatment is provided to patients who have a tooth that can be saved, however the tooth has a cavity into the nerve. This is also called a root canal. Prosthetic treatment includes crowns, fixed bridges, complete dentures, and removable partial dentures. Implants may also be an option in specific cases. When specialty dental care is recommended we are happy to help you explore those options. In the event you have a dental emergency, walk-in times are available for evaluation. You could receive same day treatment when appropriate. The Dental Department also has patient support services. Our Patient Navigator staff will help you with planning your treatment priorities, exploring financial options, and follow-up with treatment and referrals. Our Dental Insurance Support staff coordinates dental insurance benefits and pre-authorizations.

Details: Rocky Mountain Youth Clinics (RMYC) nine School-Based Health Clinics (SBHC) provide comprehensive, on-site health care services to students who do not have a medical home or are in need of health services in a convenient setting. Each SBHC is hosted by a school, and services are available to all students enrolled within the same school district as the host school. In 2015, SBHCs served more than 7,000 patients.

Details: Mission Medical Center (MMC) is here to provide health, hope, and spiritual support. Qualified patients receive integrated health services from licensed professionals in a Christ-centered environment. Mission Medical Center works with the health care community in the Pikes Peak Region to identify specialists to provide referral services on a case-by-case basis. Mission Medical Center patients receive outpatient medical and behavioral health care for chronic medical illnesses at no cost. Patients typically remain under care at MMC for 10 to 20 months. MMC coordinates continuing care with other safety-net providers, particularly Peak Vista Community Health Center. Patients typically are 19-64 years old, have no form of health care coverage, and with income below 250% of the Federal Poverty Guidelines; $29,425 for a household of one person, a family of 4 is $60,525. MMC also cares for the under-insured, people that have insurance, but high deductibles.

Convenient Care Community Clinic

Details: In 2008 Valley-Wide Health Systems, Inc. developed the convenient care model and established Convenient Care Community Clinic to give patients and the community an alternative to the local emergency room for acute and non-urgent care needs outside of traditional clinic hours. Convenient Care Community Clinic (CCCC) originated as part of the Sierra Blanca Medical Center, but was later relocated to a separate facility at 1131 Main Street in Alamosa. The CCCC model not only offers patients a convenient option for illnesses outside of regular office hours, for outpatient treatment, minor injuries, and/or acute illness, but also provides better access to walk-in, same day services which are available 7 days a week including holidays. CCCC serves as an alternative to the cost of an emergency room visit for problems on the lower end of the severity spectrum. Visits should be for acute care only and should not replace visits to your Primary Care Provider or regular physician. Refills for chronic medication prescriptions cannot be requested through this clinic. CCCC also offers FREE HIV screening services and a full spectrum of STD testing and treatment in a safe and confidential setting. CCCC does not require that you be a Valley-Wide Health Systems patient, and is open to both the community and to visitors needing services while in the San Luis Valley. After your visit, treatment or screening, our Case Managers can help coordinate follow-up care with your Primary Care Doctor or help you select a Primary Care Provider. CCCC is located within walking distance to both the Adams State University and Trinidad State Junior College Campuses and Walgreens pharmacy. CCCC accepts most insurance plans, but requires that all patients bring their insurance information and/or their insurance card.

Details: Sierra Blanca Medical Center is located on the 3rd floor of the south wing of the San Luis Valley Health Hospital, within walking distance to Adams State University. The clinic was first opened in 1996 and has undergone several changes to enhance patient care since that time. Sierra Blanca Medical Center (SBMC) provides care to the city of Alamosa, Colorado, and surrounding areas including the rural population Alamosa County, the towns of La Jara, Monte Vista, Hooper, Blanca and Fort Garland. Our clinic serves patients of various backgrounds and socio-economic statuses, including: low-income families and specialized assistance for our migrant patients. At SBMC we not only provide primary care, but we also have integrated rehabilitation/physical therapy services available. A full range of Chronic Care services are offered, such as asthma care, to provide literature and educational material so you can fully understand how to successfully manage your condition. We also offer both childhood and adult vaccinations. Being located within the hospital campus, Diagnostic, Laboratory, and Emergency Services as well as the Valley-Wide Pharmacy are easily accessible for our patients. Family Health & Planning services are also available to provide you with options based upon individual beliefs and needs. Valley-Wide Health Systems has partnered with San Luis Valley Behavioral Health Group to provide on-site Behavioral Health services, which are offered Monday through Friday. Our clinic also offers FREE HIV screening services and a full spectrum of STD testing and treatment in a safe and confidential setting. We have various doctors, physician assistants, nurse practitioners and physical therapists on hand to meet the needs of our patients including, after hours assistance, through a Telephone Triage Service. Our staff is dedicated to making sure all Valley-Wide patients receive individualized patient care, in a respectful and inclusive manner. We focus our efforts on providing excellent service, to making sure our patients feel comfortable and continue to return to our clinic for their medical needs.
