- Clinic Details: Sierra Blanca Medical Center is located on the 3rd floor of the south wing of the San Luis Valley Health Hospital, within walking distance to Adams State University. The clinic was first opened in 1996 and has undergone several changes to enhance patient care since that time. Sierra Blanca Medical Center (SBMC) provides care to the city of Alamosa, Colorado, and surrounding areas including the rural population Alamosa County, the towns of La Jara, Monte Vista, Hooper, Blanca and Fort Garland. Our clinic serves patients of various backgrounds and socio-economic statuses, including: low-income families and specialized assistance for our migrant patients. At SBMC we not only provide primary care, but we also have integrated rehabilitation/physical therapy services available. A full range of Chronic Care services are offered, such as asthma care, to provide literature and educational material so you can fully understand how to successfully manage your condition. We also offer both childhood and adult vaccinations. Being located within the hospital campus, Diagnostic, Laboratory, and Emergency Services as well as the Valley-Wide Pharmacy are easily accessible for our patients. Family Health & Planning services are also available to provide you with options based upon individual beliefs and needs. Valley-Wide Health Systems has partnered with San Luis Valley Behavioral Health Group to provide on-site Behavioral Health services, which are offered Monday through Friday. Our clinic also offers FREE HIV screening services and a full spectrum of STD testing and treatment in a safe and confidential setting. We have various doctors, physician assistants, nurse practitioners and physical therapists on hand to meet the needs of our patients including, after hours assistance, through a Telephone Triage Service. Our staff is dedicated to making sure all Valley-Wide patients receive individualized patient care, in a respectful and inclusive manner. We focus our efforts on providing excellent service, to making sure our patients feel comfortable and continue to return to our clinic for their medical needs. We treat our patients as individuals and value their safety, confidentiality and health-care needs.
