The Long Island FQHC is bringing a new kind of care to the communities that need it most. Our goal is to see that every Long Islander has a "medical home" - a place where people you know provide the kind of primary and preventative care that safeguards the health for you and your family. Great care delivered by compassionate doctors and nurses are available right in your own neighborhood. Our six health center locations provide multiple health services such as Family Medicine, Internal Medicine, Pediatrics, OB/GYN/ Dentistry, Podiatry, Optical, Behavioral Health and Nutrition. We can even help you obtain low or no-cost insurance coverage, if eligible. Accepted Forms of Payment: • Most private and managed care health insurance plans • Public health insurance (Medicare, Medicaid, Child Health Plus). • Co-payments and deductibles are due at the time of service. • Cash/Credit/Checks • Sliding Scale Discount Program for reduced rates based on your family’s size and income for qualified income-eligible insured, uninsured and under-insured patients. <p>I am not fond of the clinic. &nbsp;The staffs are unproffesional and seems to only speak spanish to one another in your presence. The clinic itself is in bad shapes and no one seem to mind. Compared to the doctors' who are pretty nice, the staffs are very rude and do not know how to give a proper answer to a correct question. Not sure how they got the job, but i am pretty sure they are not qualified. &nbsp;</p> <p>&nbsp;</p>
