Clinic Details:Our Albany Park community health center is our newest location. This health center will provide 4,000 new individual patients with primary care medical visits, behavioral health visits, and oral health screenings for children ages 0-4.

Details: Erie Helping Hands is located in Albany Park and provides a wide range of primary health care and outreach services to enhance the health of the Albany Park community. You can come to our Helping Hands Health Center for everything from free pregnancy tests and school physicals to cholesterol checks and diabetes management. We even offer laboratory services and medication.
Details: Erie Foster Avenue Health Center is Erie’s first location on a hospital campus. Located at the corner of Foster and California, Erie Foster Avenue offers all new equipment in a state-of-the-art, beautiful environment. We offer primary medical care for children, adults and older adults, prenatal care and women’s health services, behavioral health services, chronic disease management, vaccines and flu shots, STI testing and treatment. It also hosts a Lending Hands for Life satellite clinic, which provides care for patients with HIV/AIDS. Erie Foster Avenue also has a five-chair, state-of-the-art dental facility that provides comprehensive oral health services - from cleanings and fluoride treatments to crowns and root canals - to children and pregnant women. Adult dental patients also must be medical patients of Erie Family Health Center. As Erie’s first location on a hospital campus, Erie Foster Avenue patients will also have access to the on-site specialty care services provided by Swedish Covenant Hospital.
Details: Our Lincoln Square community health center opened its doors in the spring of 2009. We provide a medical home to some 4,000 patients in the Lincoln Square, Albany Park and Irving Park community areas.
Details: Our Dental Services provide comprehensive dental treatment and restorative services. We help patients achieve optimum oral health, enhance their appearance and increase their confidence. The Dental Services Program helps homeless individuals maintain or attain dental wellness through free dental services provided by volunteer dentists and hygienists. There are very few sources of dental care available to people who are homeless. Exhibiting good dental hygiene and being pain free are essential to becoming employed and maintaining stable employment. Through our partnership with University of Illinois at Chicago-College of Dentistry, we have established a nationally-recognized, first ever Student-Run Dental Clinic. Our Dental Clinic provides a hands-on experience in the field of public health dentistry to over 200 students in the last three academic years. We have provided treatment to over 2500 homeless adults. We restore their oral health and free them of mouth pain. Our services have provided them with the tools and the knowledge needed to properly maintain their oral health as well as instruct their family in the future.
Details: At the federally funded AHS Family Health Center, we offer comprehensive services for individuals in all stages of life. We emphasize maternal health, pediatric services, women’s health, adolescent care, internal medicine for adult men and women and services for senior citizens.
Details: The Indian American Medical Association (IL) Charitable Foundation (IAMACF), is a 501(c)(3) nonprofit primary care healthcare community clinic located at 2645 W. Peterson Avenue, in the heart of Chicago's West Ridge community. The West Ridge community is home to one of the most culturally diverse communities, as it is a gateway to numerous immigrant and refugee populations in Chicago. The clinic serves over 3000 people annually with operating hours on Saturday and Sunday from 10:00 am to 2:00 pm. Registration for patients opens one hour prior to opening on the weekends (9:00 AM). Patients are seen on a first come first served basis and does have the ability to take appointments. It is run by volunteer physicians, including members of the Indian American Medical Association of Illinois, and medical students coordinated by New Life Volunteer Services. The free clinic serves low-income populations, who are uninsured and underserved in that have no affordable or culturally competent medical facility for their healthcare need. The IAMACF community clinic provides a culturally appropriate environment, with volunteer physicians and volunteers who also extend free (limited) pharmacy and lab services to walk-in clients. The IAMACF is also a teaching clinic for medical students from six of our area's medical schools, including the Chicago Medical School, Loyola University, Northwestern University, Rush University, University of Chicago and University of Illinois at Chicago. Details: The New Life Volunteering Society free health clinic serves as a comprehensive medical care facility dedicated to serving the uninsured of Chicago in primary and secondary prevention of chronic medical conditions and in the training of future health care professionals by providing an educational setting to learn and practice clinical skills.
Details: Mercy to Mankind Clinic, NFP, is a volunteer-driven neighborhood clinic that partners with our providers and other organizations in an effort to address the health needs of our community. Guidance for our healthcare ministry is fulfilled though Rahmat E Alam Foundation, providing spiritual support for the community.
