Clinic Details:Our Lincoln Square community health center opened its doors in the spring of 2009. We provide a medical home to some 4,000 patients in the Lincoln Square, Albany Park and Irving Park community areas.

Details: Erie Foster Avenue Health Center is Erie’s first location on a hospital campus. Located at the corner of Foster and California, Erie Foster Avenue offers all new equipment in a state-of-the-art, beautiful environment. We offer primary medical care for children, adults and older adults, prenatal care and women’s health services, behavioral health services, chronic disease management, vaccines and flu shots, STI testing and treatment. It also hosts a Lending Hands for Life satellite clinic, which provides care for patients with HIV/AIDS. Erie Foster Avenue also has a five-chair, state-of-the-art dental facility that provides comprehensive oral health services - from cleanings and fluoride treatments to crowns and root canals - to children and pregnant women. Adult dental patients also must be medical patients of Erie Family Health Center. As Erie’s first location on a hospital campus, Erie Foster Avenue patients will also have access to the on-site specialty care services provided by Swedish Covenant Hospital.
Details: Erie Helping Hands is located in Albany Park and provides a wide range of primary health care and outreach services to enhance the health of the Albany Park community. You can come to our Helping Hands Health Center for everything from free pregnancy tests and school physicals to cholesterol checks and diabetes management. We even offer laboratory services and medication.
Details: Erie Amundsen School Based Health Center is dedicated exclusively to adolescents in the Chicagoland area. At Erie, we know that being a teenager isn’t always easy so we are here to help you with a wide variety of issues - from school and sports physicals to low-cost contraception to pregnancy tests. Our services are friendly, respectful and confidential, so you’ll never have to worry about seeking out the care or help you need.
Details: Our Dental Services provide comprehensive dental treatment and restorative services. We help patients achieve optimum oral health, enhance their appearance and increase their confidence. The Dental Services Program helps homeless individuals maintain or attain dental wellness through free dental services provided by volunteer dentists and hygienists. There are very few sources of dental care available to people who are homeless. Exhibiting good dental hygiene and being pain free are essential to becoming employed and maintaining stable employment. Through our partnership with University of Illinois at Chicago-College of Dentistry, we have established a nationally-recognized, first ever Student-Run Dental Clinic. Our Dental Clinic provides a hands-on experience in the field of public health dentistry to over 200 students in the last three academic years. We have provided treatment to over 2500 homeless adults. We restore their oral health and free them of mouth pain. Our services have provided them with the tools and the knowledge needed to properly maintain their oral health as well as instruct their family in the future.
Details: Our Albany Park community health center is our newest location. This health center will provide 4,000 new individual patients with primary care medical visits, behavioral health visits, and oral health screenings for children ages 0-4.
Details: Community Counseling Centers of Chicago (C4) offers a comprehensive range of services designed to help adults, children and families cope with varied behavioral health challenges. C4 views treatment as a partnership with the individual and works to provide a supportive network that enables an individual to heal in a trusted and professional environment, In addition to the primary care services Heartland Health Centers offers, services include counseling, crisis intervention, social services, psychiatry, and client advocacy.
Details: The New Life Volunteering Society free health clinic serves as a comprehensive medical care facility dedicated to serving the uninsured of Chicago in primary and secondary prevention of chronic medical conditions and in the training of future health care professionals by providing an educational setting to learn and practice clinical skills.
