Our pediatric, adult and family medicine practitioners provide comprehensive primary and selected specialty health care services. The UVM Health Network-CVPH Diagnostic Center located within our health center offers a range of ancillary services including x-ray, EKG and blood draw. In addition, to maintain high-quality care in our community both now and in the coming years, the staff at Champlain Family Health is dedicated to educating future health care providers.

Details: Asbury First United Methodist Church and the University of Rochester School of Medicine and Dentistry have been providing free access to urgent care, work physicals, and health maintenance to the Rochester community for 10 years. The clinic is run by volunteers (church members, nurses, medical students, pharmacy students, and physicians) and is dedicated to providing compassionate, professional, and quality care to uninsured and underinsured patients.
Details: The Einstein Community Health Outreach (ECHO) Free Clinic is a collaboration between the Institute for Family Health and the Albert Einstein College of Medicine. The clinic provides free, high-quality, comprehensive health care to the uninsured population of the Bronx of New York City. The ECHO Free Clinic embraces the spirit of volunteerism and service embodied in our health care professionals and student volunteers.
Details: Hudson Headwaters physicians provide primary care to patients in nearby adult care facilities, nursing homes and rehabilitation centers. These partnerships make high-quality health care available to you and your loved ones through different stages of life.
Details: The Adult Free Clinic at UHS Primary Care Binghamton continues the longstanding tradition of the former SUNY Upstate Medical Center Dr. Garabed A. Fattal Community Free Clinic which has now been relocated the UHS Primary Care - Binghamton.
Our clinic treats adult (ages 18 and older) patients who are unable to obtain medical insurance or otherwise afford their medical care. All patients are screened for eligibility as, in some cases, our counselors can assist patients in obtaining medical insurance as well.
Patients are cared for by licensed, board-certified internists as well as UHS Internal Medicine resident physicians and SUNY Upstate medical students in training under direct supervision. Records are kept in our secure, state-of-the-art Epic electronic health record.
Services Offered
Patients may be seen for general physical exams, wellness screenings, vaccinations, treatment for chronic conditions, both men’s and women’s health concerns, limited specialty referrals, medication management, as well as for some labs and imaging tests. Prescription medications are available at the neighboring UHS Pharmacy Summit Building based on our cost-free supply list.

Walk-Ins are welcome for established patients.

Details: ConnextCare Parish is a patient-centered health care practice providing Oswego and Northern Onondaga County residents with a variety of comprehensive health care and related services. The center is operated by ConnextCare, a private, federally funded non-profit organization governed by a volunteer Board of Directors.
