Clinic Details:Parkview OB/GYN was purchased by Delaware Valley Community Health, Inc. in September 2007. Building on over 25 years of providing health care to women in the Juniata Park section of North Philadelphia, it became the fourth practice site of DVCH, Inc. Podiatry services were added in July 2008. All patients have access to other health services such as Adult Medicine, Pediatrics, Dental, Social Work and Health Insurance Enrollment at our closely located Maria de los Santos Health Center at 401 W. Allegheny Avenue by walking in or calling 215-291-2500 to schedule an appointment.
Details: Through a partnership with Girard Medical Center, DVCH opened a satellite practice on the Girard’s campus in 2016. Fairmount Primary Care Center at Girard brings our comprehensive primary care medical services to the patients of Girard Medical Center, furthering our commitment to making healthcare accessible to patients of all needs.
Details: Annex West Health Center is operated as a partnership between Action Wellness (formerly Action AIDS) and FPCN. The health center opened in January 2015 and is based on the patient-centered medical home model, which utilizes a team approach to foster communication between providers and patients, increase care coordination, and enhance overall quality of care. At Annex West, FPCN Nurse Practitioners work with Action Wellness medical Case Managers to provide services including: ongoing individualized care tailored to the patient’s needs, access to a behavioral health consultant, linkage to ongoing behavioral health services, vocational services, and housing information services. Annex West has a robust linkage with prison systems in order to serve people reentering society. In addition, staff are competent in issues related to caring for transgender patients.
Details: In partnership with Prevention Point Philadelphia and the Norris Square Civic Association, Clinica Bienestar (Well Being Services), is one of the city’s first Spanish-speaking HIV clinics. The primary care clinic is supported by the extensive medical, case management and social services of Philadelphia FIGHT. Clinica Bienestar (Well-being Clinic) is a comprehensive HIV primary care clinic created through a partnership between Philadelphia FIGHT Community Health Centers and Prevention Point Philadelphia (PPP). It is located within PPP’s Harm Reduction Services Center at 2913-15 Kensington Avenue in North Philadelphia. Clinica Bienestar staff are especially skilled in providing medical and social services to individuals who are actively using or have a history of using injection drugs and have received inconsistent HIV medical care. The medical providers, medical case management, clinical staff, and outreach workers are bilingual in English and in Spanish. Services provided at Clinica Bienestar include HIV primary care, hepatitis C treatment, HIV PrEP, wound care, medical case management, medication-assisted treatment, and harm reduction services.
Details: In partnership with Broad Street Ministry and their hospitality collaborative, FIGHT Community Health Centers Clinic at Broad Street Ministry is a satellite site of John Bell Health Center offering primary care to the guests of Broad Street Ministry. The satellite clinic offers primary medical care provided by Philadelphia FIGHT, and an onsite Care Coordinator provided by Health Partners. This groundbreaking partnership help provided access to quality health care for individuals who are struggling with homelessness and chronic health conditions. The clinic is located within the drop-in space at Broad Street Ministry at 315 S. Broad Street. The medical and support staff are trained in providing culturally competent care to the homeless population and the guests of BSM. We are active members of the National Health Care for the Homeless Council to ensure we are current in best practices for the community. Our staff includes a primary care provider, a mental health care coordinator, a program manager, and a medical assistant.

Details: Abbottsford Falls is the oldest of FPCN’s locations. It began as two separate sites in 1992 and 1994 both located within nearby public housing communities, and then eventually merged, was renamed and moved to its current space on the East Falls/Germantown line. Abbottsford Falls provides comprehensive healthcare services, including Primary Care with integrated Behavioral Health, Outpatient Behavioral Health, Dental Care, Social Work, Outreach & Enrollment, Diabetes and Nutrition education and support, Mind/Body practices, prenatal care, medical specialty services, physical therapy, on-site pharmacy, transportation assistance and a fitness room. It underwent a renovation and expansion in 2007 in order to increase its capacity to serve even more patients. The site has a longstanding community advisory board whose composition includes some of the Network’s longest standing, first patients.
