Details: Our dental clinic offers emergency, preventative, and restorative dental care to people who may be uninsured, underinsured, low income or homeless, and may otherwise go without care. Services provided include: filling, crowns, endodontics, extractions, periodonture, bridges, prophylaxis, application of fluoride and sealants and oral health education. We accept OHP/Trillium and private dental insurance. Uninsured patients are charged on a sliding scale based on income. Details: The mission of the Community Health Centers of Lane County, a federally qualified health center (FQHC), is to improve the health and wellness of our community through access to affordable, holistic healthcare. We have adopted a model of primary care called Patient Centered Medical Home. This model of primary care prioritizes the patient, who has increased access to other services beyond general medical care at our clinics. We coordinate our patients' health care across the community, among specialists, emergency and hospital visits, diagnostic testing services, and other services. Details: Chrysalis is a client-centered, humanistic treatment program based on the belief that all people can develop a healthy, drug-free lifestyle. We provide innovative outpatient services for adults over 18 who need alcohol and/or drug abuse treatment as well as mental health services.
Details: NEST is designed to help adults experiencing homelessness navigate available resources and sign up to receive assistance. Services offered may include assistance in accessing primary medical care, case management and advocacy, outpatient drug and alcohol treatment, group therapy, drug detox, mental health counseling and evaluation, SSI/SSDI assistance from a public benefits specialist, and transportation to appointments.
