- Details: The SMCS community health center network consists of nine federally-qualified health center locations, which include walk-in care with no appointment necessary at our Ludlow, Londonderry, Rockingham, and Springfield, VT; and Charlestown, NH locations. These sites make high-quality, comprehensive, and affordable care available to all, based on a sliding fee scale. Services include primary and preventive health for people of all ages, pediatrics, diagnostic lab and radiologic services, health screenings, mental health programs, and access to discounted pharmaceuticals. Services also include dental care at the Ludlow Dental Center and Chester Dental Center.

- Details: On-site services include: primary and preventive care, family medicine, pediatrics, outpatient mental health counseling, minor skin surgeries, and planned diabetic visits. Lab and digital x-ray services are offered on-site and aid us in providing you prompt diagnosis and treatment. Free blood pressure checks are always available as needed. If you have an unexpected illness, same day appointments are available. Please call as early in the day as possible so we can make every attempt to accommodate your needs. If you wish to utilize our walk-in service, it is available seven days a week with no appointment is necessary. We will make every effort to meet your needs in a timely manner. Referrals to medical specialists and dental and vision care will be coordinated through your medical home with consultation between you and your primary care clinician.
