Details: Community Health is a primary care network that provides nationally-recognized programs, a focus on wellness, dental, behavioral health and pediatric specialties, walk-in Express Care, a culture of community and quality health care that almost everyone, insured or uninsured, has come to depend on. Community Health, based in Rutland, is the largest of the 12 federally qualified health centers (FQHC) in the state of Vermont. Nationally, more than 1,200 FQHCs provide primary health care to one out of every 14 people. In Vermont, FQHCs provide care to approximately one in four Vermonters. Community Health opened in 2004 as Community Health Centers of the Rutland Region providing a network of medical, dental and behavioral health care. It’s grown to include seven medical facilities and a pharmacy – Rutland, Brandon, West Pawlet (Mettowee), Shorewell, Castleton, Community Health Pediatrics, Community Health Dental, Brandon Community Pharmacy – which in 2019 handled close to 187,900 patient visits, providing care to over 41,800 patients in the Rutland County/southern Addison County region which includes over three-quarters of the residents of Rutland County. First established in 1965 as part of President Lyndon Johnson’s War on Poverty, FQHCs are primary care centers, located in underserved and rural areas, that provide high quality, cost-effective health care regardless of an individual’s ability to pay. The success of the FQHCs was recognized by Congress when creating the Affordable Care Act (ACA) in 2010 and the ACA legislation permanently authorized the program.
Details: The Kingwood Health Center is conveniently located just a half mile off Interstate 89 on Route 66 west in Randolph. Physical therapy staff works to relieve pain and improve functional abilities to return patients to life and work following an injury, illness or surgery. The department also includes the Continence Program and the Balance Center. The Balance Center provides specialized physical therapy for balance disorders, such as vertigo. This therapy is also known as vestibular rehabilitation. The Continence Program treats urinary and bowel incontinence. Aquatics therapy is additionally available at Vermont Technical College’s nearby SHAPE facility.
Details: The SMCS community health center network consists of nine federally-qualified health center locations, which include walk-in care with no appointment necessary at our Ludlow, Londonderry, Rockingham, and Springfield, VT; and Charlestown, NH locations. These sites make high-quality, comprehensive, and affordable care available to all, based on a sliding fee scale. Services include primary and preventive health for people of all ages, pediatrics, diagnostic lab and radiologic services, health screenings, mental health programs, and access to discounted pharmaceuticals. Services also include dental care at the Ludlow Dental Center and Chester Dental Center.

Details: On-site services include: primary and preventive care, family medicine, pediatrics, outpatient mental health counseling, minor skin surgeries, and planned diabetic visits. Lab and digital x-ray services are offered on-site and aid us in providing you prompt diagnosis and treatment. Free blood pressure checks are always available as needed. If you have an unexpected illness, same day appointments are available. Please call as early in the day as possible so we can make every attempt to accommodate your needs. If you wish to utilize our walk-in service, it is available seven days a week with no appointment is necessary. We will make every effort to meet your needs in a timely manner. Referrals to medical specialists and dental and vision care will be coordinated through your medical home with consultation between you and your primary care clinician.
