    • Details: Shenandoah Community Health (SCH) is a Federally Qualified Community Health Center and Primary Care Medical Home that offers access to primary care, behavioral health, women’s health, and oral healthcare right here in the Shenandoah Valley of West Virginia and Virginia. Since 1976, our dedicated clinicians, nurses, and support staff have worked to ensure that our patients receive the highest quality, most comprehensive healthcare possible. SCH’s wide range of coordinated healthcare services includes routine care as well as chronic disease treatment and management; mental health and substance use treatment; specialized care for women including prenatal care and delivery, gynecology and surgery; and comprehensive dental care for children and adults. As a medical home, SCH also offers WIC Nutrition Programs; HIV case management; weight and wellness program; on-site pharmacy; lab and x-ray services; nursing outreach to migrant and seasonal farmworkers and the homeless; medication assistance; assistance with health insurance enrollment; social work services; and family support resources. Health education classes, a breastfeeding support group and diabetes management program are offered free of charge and are open to the public. Shenandoah Community Health serves all patients - adults and children - regardless of ability to pay. We also provide interpretation services for patients whose primary language is not English.

    • Details: Shenandoah Community Health’s Special Populations Department has over 45 years of combined experience coordinating visits to migrant camps and sites where the homeless gather. Our outreach team consists of nurses, bilingual outreach workers, and drivers who support this high risk and underserved population. Special Populations’ goal is to reduce barriers to care, ensure access to care and improve services and coordinated healthcare by collecting documentation such as nursing assessments, demographics, and health screening intakes from migrant camps, homeless and other outreach sites to populate an electronic health record for each patient. We provide free nursing assessments, case management, health education, as well as referrals for medical, dental, and behavioral healthcare to agricultural workers and the homeless. We also work with the growers, health departments, churches, shelters and homeless coalitions to help identify anyone who could benefit from our services. To reduce barriers to care, Shenandoah Community Health’s Special Populations Department provides: free interpretation services in Spanish, French, and Haitian Creole at the health center, the fields of agricultural workers, in the shelters, and sites where homeless gather; telephone interpretation services and translation of documents in Spanish; patient advocacy and assistance with sliding fee, prescriptions, billing, laboratory and x-ray services; free transportation services for agricultural workers and their families, and homeless who do not have transportation, to get to our center or referral sites; free on-site health education in English, Spanish and Haitian Creole to migrants on topics such as ergonomics, first aid, and sexually transmitted diseases. We provide outreach to ensure access to healthcare at 28 migrant camps located throughout Berkeley, Jefferson, and Morgan Counties in West Virginia, Winchester, Virginia and adjoining counties, and Big Pool, Washington County, Maryland. In addition, Special Populations provides outreach to seven sites where homeless gather in Martinsburg, WV.

    • Details: Shenandoah Community Health (SCH) is part of The Ryan White Outpatient Program, which identifies people who are at risk for HIV and/or affected by the disease. The program offers comprehensive healthcare testing to individuals living with HIV regardless of their ability to pay. In 2003, SCH was designated a Ryan White - Part C Program location. This satellite program is sponsored by the Robert C. Byrd Health Sciences Center of West Virginia University and serves Berkeley, Grant, Hampshire, Hardy, Jefferson, Mineral, Morgan, and Pendleton Counties. In 2010, SCH was designated a Ryan White - Part A Program location enabling SCH to increase service categories to Berkeley County as well as Jefferson County residents. This program is sponsored by the DC Department of Health and care is supervised by WVU Positive Health Clinic in Morgantown. The SCH | Ryan White Program is one of 27 institutions nationwide awarded Federal Ryan White Care Act funding.

    • Details: In partnership with the State of West Virginia, our WIC program is ready to assist young families in giving their infants, toddlers, and preschoolers a healthy start.
