Details: Kids First Health Care is committed to improving the health and well-being of infants, children, and youth through providing primary and preventive health services in partnership with schools and other community organizations.
Details: School-Based Health Centers are offered in partnership with the 9R School District in La Plata County to provide accessible healthcare on site, designed to keep students healthy, in school and ready to learn. Our clinics are located at Durango High School and Florida Mesa Elementary School. All La Plata County students up to age 21 may enroll and access services at either one of the clinics. This includes home-schooled youth, online learners, charter school and private school students. In addition, siblings of students ages 2 to 21 are also welcome.
Details: Denver Health’s School-Based Health Centers (SBHCs) offer specialized pediatric focused care to treat most health conditions that affect school-aged children. Our services are available throughout the school year to any Denver Public Schools (DPS) student or child that attends a DPS affiliated Early Childhood Education (ECE) center within Denver County. SBHCs offer convenient care that limits the amount of time students are out of class and parent/guardians have to be off work. In many cases, students are able to get same day or next day appointments.
Details: Salud is a Federally Qualified Health Center (FQHC) with twelve locations and a mobile unit. We provide medical, dental, pharmacy and behavioral health care services to keep you and your family healthy. We focus on low-income, medically underserved populations as well as the migrant and seasonal farmworker population. Since 1970, Salud has been firmly committed to providing care to all community members regardless of finances, insurance coverage, or ability to pay. We offer a sliding fee scale based on family size and income. Insurance enrollment assistance is available. Salud is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
Details: Our Health Center at Printers Parkway, 340, is home to Peak Vista's Clinical Training program – the Family Medicine Residency (FMR) program; our Health Center at Printers Parkway, 350, is home to Peak Vista's Clinical Training program - Advanced Practice Registered Nurse (APRN) Primary Care Fellowship. It is designed to offer you high-quality, comprehensive primary medical and behavioral health care. We care for people of all ages and offer a broad range of services – from preventive routine exams to same-day treatment of acute illness and injury, on-going primary care and follow-up for chronic health conditions. Our health centers are also home to a number of volunteer specialty physicians who treat our patients with needs for specialty care and have been referred by primary care providers. Should you require specialty care, we are happy to refer you to other qualified community providers.
