About this clinic

Due to the high demand of health care need and access to different areas in the San Luis Valley, Valley-Wide Health Systems, Inc. purchased the clinic in Moffat in 1996, and renamed it the Moffat Family Health Center. This supplied health care services to the northern area of the San Luis Valley, including Salida, Crestone, Center, Saquache, and Mosca. By choosing the Moffat Family Health Center (MFHC) as your medical home you will receive comprehensive quality care. Let us care for your whole family to meet all of your health care needs. MFHC provides case management and behavioral health services along with a wide scope of medical services. MFHC also offers FREE HIV screening services and a full spectrum of STD testing and treatment in a safe and confidential setting. MFHC offers pharmacy services to established Valley-Wide patients.
