Street Level Health Project is an Oakland-based community center dedicated to improving the wellbeing of underinsured, uninsured, and recently arrived immigrants in Alameda County.

  • Details: Since 2000, we have been providing high quality, youth-friendly services that include medical care, reproductive health, sports physicals, immunizations and management of acute and chronic conditions to Oakland Technical High School students. We also serve students enrolled at nearby Oakland International High School and recent graduates from both schools up to the age of 21. In addition, we provide individual and group counseling, and case management services. Patients are seen by a clinical health educator who conducts one-on-one appointments that cover issues such as sexual health, birth control, and nutrition. The health educator conducts classroom presentations and coordinates a Peer Health Education Group and helps students develop presentation and leadership skills. We are passionate about improving the health and wellness of the students in our school community.
• Tiger Health Clinic (fremont Wellness Center)

  • Details: The Fremont Wellness Center opened its doors in 2000. At the Fremont Wellness Center, we use an integrated care model to provide holistic comprehensive support for our patients. We serve current students at Fremont High School and recent graduates up to age 21. We provide medical services including reproductive health, prevention and treatment of sexually transmitted infections (STIs), sports physicals, and immunizations. Patients are also seen by a clinical health educator who conducts one-on-one appointments that cover issues ranging from sexual health to nutrition. In addition, we offer behavioral health services to students through an onsite behavioral health specialist. We also support community health education through the Fremont Youth Wellness Advisory Board (YWAB), a group comprised of peer health educators who are passionate about changing and improving the health of their school community.

  • Details: La Clínica’s HIV prevention department, Together Reaching Users Combating HIV and AIDS (TRUCHA), has been delivering HIV testing, outreach, and prevention services for over 25 years. TRUCHA was established 25+ years ago to respond to the threat of HIV in the Latinx community, especially for those most vulnerable, such as the jornaleros (day laborers). Since then, TRUCHA has grown and undergone many changes. Today TRUCHA is still a site where people in the community can come for culturally relevant services related to HIV prevention and HIV care, as well as other services that affect our community. The prevention program consists of staff members who are certified HIV test counselors, trained in numerous elements of behavioral change, health education, and health communication, and are culturally and linguistically representative of the community they serve. TRUCHA staff includes HIV Test Counselors, PrEP Navigators, and Health Educators who lead support groups and group-level intervention programs. TRUCHA further specializes in linking/re-linking clients living with HIV to HIV care. Staff members employed by La Clínica’s HIV prevention program are bilingual in English and Spanish, bicultural, state-certified HIV test counselors, trained in numerous elements of behavioral change, health education, and health communication, and experienced working in the Latinx LGBTQ+ community. TRUCHA’s HIV prevention staff also assists in referring patients from TRUCHA to other services offered at La Clinica.

  • Details: At UCSF Benioff Children's Hospital, we're experts in taking care of the health needs of children from birth through adolescence. We firmly believe that the best way to help a child stay healthy is through regular checkups, preventive medicine and early intervention. That's what we do at our Pediatric Primary Care Clinic. Our doctors and nurses provide routine preventive care – including checkups, vaccinations, vision and hearing screenings - and diagnose and treat the many minor illnesses and injuries kids experience. They'll work with you to help manage chronic conditions and provide referrals to a pediatric specialist when needed. They take the time to get to know your child and your family and to answer all of your questions about your child's health.
