Clinic Details:La Jara Dental Clinic was established in 1997 to serve the residents of Conejos and neighboring Costilla counties with quality dental care. La Jara Dental Clinic (LJDC) is located in the Town of La Jara in Conejos County. In 2005 LJDC received a significant renovation to better accommodate dental care needs. LJDC provides General Dentistry services. Our goal is to provide you with information to guide you through your dental health decisions and comprehensive care. We offer diagnostic services like routine exams, x-rays, and dental screening. Restorative care includes both composite and amalgam fillings depending on patient preference and suitability. Preventive care includes routine cleanings, oral hygiene education, fluoride application, and sealants to prevent decay in deep grooves of the molars. We treat the whole family; including babies, children, adolescents and adults. Pediatric services we provide include restorations, space maintenance, preventive care, and Headstart screenings. Sometimes patients will need more than routine care. Periodontal treatment includes deep cleanings called Scaling and Root Planning to stop bone loss from hard deposits and bacteria below the gum. We also offer Oral Surgery services like extractions, surgical extractions, and alveoloplasty (smoothening the bone to prepare for a denture). Endodontic treatment is provided to patients who have a tooth that can be saved, however the tooth has a cavity into the nerve. This is also called a root canal. Prosthetic treatment includes crowns, fixed bridges, complete dentures, and removable partial dentures. Implants may also be an option in specific cases. When specialty dental care is recommended we are happy to help you explore those options. In the event you have a dental emergency, walk-in times are available for evaluation. You could receive same day treatment when appropriate. The Dental Department also has patient support services. Our Patient Navigator staff will help you with planning your treatment priorities, exploring financial options, and follow-up with treatment and referrals. Our Dental Insurance Support staff coordinates dental insurance benefits and pre-authorizations.
Details: As one of only a few federally-qualified Community Health Centers on the Western Slope, UMC is dedicated to ensuring that all patients receive quality healthcare regardless of their ability to pay. We work with patients to determine what they can afford, establish relationships with specialists, and enroll them in programs to offset costs. At the Uncompahgre Medical Center, we are sensitive to rising healthcare costs, including that of healthcare insurance, which can be priced out-of-reach for some families.
Details: Since 1978, as others closed their doors to those without private health insurance, we have continued to open ours. From a single clinic we have now expanded into a network of health care facilities along the I-70 corridor from Parachute to Avon, and along the Roaring Fork Valley to Basalt. As a nonprofit, community-led, Federally Qualified Health Center system, we are dedicated to providing comprehensive, integrated health care for all. We firmly believe health care is a basic human right, not a privilege. Mountain Family currently serves more than 18,000 patients. Our system integrates medical, dental and behavioral care - a necessary measure for the vulnerable, who often show up in our clinics with medical problems related to poor teeth, substance abuse, trauma, depression, anxiety and other complicated combinations. We exceed national clinical outcome goals for childhood immunizations; hypertension control; tobacco screening and cessation; healthy weight screening, nutrition and activity counseling; depression screening and follow-up; early entry into prenatal care; and healthy birth weight babies. We were also recently rated Colorado’s top performing community health system for diabetes control.
Details: Metro Community Provider Network (MCPN) is a 501(c) (3) Federally Qualified Health Center (FQHC) with a 27-year history of providing primary care and other health-related services in a community-based setting. Established in 1989 in response to the primary health care needs of the medically indigent population in suburban Denver, MCPN has the largest population base of any FQHC in Colorado and provides primary care, dental, pharmacy, behavioral health, health education and outreach and enrollment services in accordance with its mission. MCPN’s current service area encompasses Adams, Arapahoe, Douglas, Jefferson, and Park counties and the Cities of Aurora, Arvada, Edgewater, Englewood, Lakewood, Littleton, Parker, and Wheat Ridge in the state of Colorado. From a single clinic in 1989, MCPN has grown into a large, integrated health care delivery system with more than 20 health center locations providing primary medical care, dental care, behavioral health, and wrap around services, including three School Based Health Centers (SBHCs) and a mobile dental van. In 2016, MCPN opened three additional locations within the service area, which will allow the organization to substantially increase service capacity. MCPN addresses the need for affordable and accessible medical, behavioral health, and dental care among low-income, uninsured, and underserved populations residing outside the city and county of Denver.
