Details: Near North Health Service Corporation (NNHSC), a 501(c)(3) non-profit Federally Qualified Health Center (FQHC), is one of the largest providers of community-based primary health care in Chicago. We provide medical services, social services, and nutrition education to medically indigent and uninsured individuals and families. Our philosophy reflects an approach that anticipates community needs and plans for expansions that are developed in accordance to these needs. Details: For more than 25 years, ACCESS has been on the frontlines of community-based health care. ACCESS provides a continuum of care model that connects patients to health care resources both within and beyond the walls of our 35 federally-qualified health centers (FQHCs). ACCESS' services are designed to address the health of our underserved communities in such areas as preventive care, chronic disease management, and support services. To address patients' comprehensive health needs, ACCESS physicians, nurse practitioners, midwives and other providers are teamed with outreach staff, case managers, social workers and substance abuse counselors to advance a continuum of care.

Nurse Practitioners promote a comprehensive approach to health care and emphasize the overall health and wellness of their patients. MSHC Humboldt Park is fully equipped for those in the community with physical and mental health disabilities. We have state-of-the-art equipment that makes our clinic fully accessible for all people, including those who use wheelchairs or have physical limitations. Our staff also specializes in managing the healthcare needs of people living with chronic mental illness. The care you receive at MSHC Humboldt Park is directly connected to the entire UI Health System including a hospital, outpatient clinics, and specialty care.
Details: Erie Humboldt Park Health Center is our third largest health center and provides a wide variety of services for men, women and children - including adult primary care, women’s health care, behavioral care, pediatric care, prenatal care and outreach to enhance the health of the Humboldt Park community. It is also the home of Lending Hands for Life, one of the city’s oldest and largest comprehensive treatment programs for HIV/AIDS. You can come to our Humboldt Park health center for everything from free pregnancy tests and school physicals to cholesterol checks and diabetes management. We even offer counseling services to help you deal with family problems or emotional issues. Erie Humboldt Park also has a five-chair, state-of-the-art dental facility that provides comprehensive oral health services - from cleanings and fluoride treatments to crowns and root canals - to children and pregnant women. Adult dental patients also must be medical patients of Erie Family Health Center. It’s easy and free to get tested for HIV without an appointment at Erie Humboldt Park!
Details: PCC Salud Family Health Center moved into a newly renovated facility on April 8, 2013. The facility includes a large patient waiting and registration area, newly renovated exam rooms, offices and restrooms, as well as new windows, flooring, lighting, plumbing, roof, heating and cooling system, and building entrances with signage. As a result of the renovations, PCC Salud Family Health Center meets the requirements for Leadership in Energy and Environmental Design (LEED) silver certification from the U.S. Green Building Council.
